Directions

1. Print the Sample Recreation Center Assessment
2. Share sample assessment with your Wellness Team (i.e., School Health Council) and complete the sample assessment together, and decide if your setting will apply to receive a nrgBalance Zone Membership Award
3. During the open enrollment period (Feb. 2 - March 31, 2009), complete the online enrollment form and print a copy for your files
4. Upon completion of enrollment, apply for a nrgBalance Zone Membership Award to help implement your healthy change goal for the coming year
5. NEW! Access member only resources to help promote healthy eating and physical activity in your setting
